慢性腎臓病患者の栄養管理と蛋白質制限に関する近年の動向 Recent Developments in Nutritional Management and Protein Restriction of Patients with Chronic Kidney Disease 香山, 真衣子 Kayama, Maiko 香山 Kayama 真衣子 Maiko CKD 栄養 蛋白質 CKD Nutrition Protein 透析患者を含む慢性腎臓病(CKD)患者には、蛋白質制限を始めとした栄養管理が必要であるが、蛋白 質の過剰な制限による弊害も問題となっている。CKD 患者の栄養管理に関する近年の動向についてレビ ューを行い、必要な支援と今後の課題について検討した。その結果、透析導入前のCKD 患者の栄養管 理では、腎機能低下を抑制するための蛋白質制限の必要性と、患者のアドヒアランスを向上させ、食事 管理を患者が意欲的に行うための個別的介入の重要性が示された。また、透析患者の栄養管理では、蛋 白質制限によるリスクと、栄養状態を良好に保ち合併症を予防する必要性が示された。 いずれの段階においても、多職種による介入は重要であり、透析導入前では、患者が蛋白質制限をし ながらでも栄養状態を良好に保つことができるように、透析導入後では、口腔の健康を維持してサルコ ペニアやフレイルを予防し、健康寿命の延伸ができるように支援していく必要があると考えられた。 Protein restriction is necessary in the nutritional management of patients with chronic kidney disease (CKD), including dialysis patients, but side effects from excessive protein restriction are also problematic. This article is based on a review of recent developments in the nutritional management of patients with CKD and discusses needed support and future challenges. The results indicate that protein restriction is necessary in the nutritional management of CKD patients prior to the introduction of dialysis in order to reduce the decline in renal function and the importance of individualized interventions to improve patient adherence and patient motivation for dietary management. And the nutritional management of dialysis patients showed the risks associated with protein restriction and the need to maintain good nutritional status and prevent complications.
