歯の喪失を含め、加齢による口腔の器質的変化は、摂食嚥下や構音等の口腔機能に影響を及ぼすことが知られている。一方で、歯を喪失しても義歯を装着することで良好な咬合関係を維持できれば、口腔機能や精神的な満足を得ることができ、QOLの向上に有効である等、義歯が及ぼす全身への影響も報告されている。
義歯が及ぼす口腔機能への影響については、口腔機能低下症の患者に義歯の適合状態を改善し、同時に口腔健康管理を実施したところ、口腔機能が改善したとの報告がある。本研究では、義歯の使用自体が口腔機能の維持に影響を及ぼすのか明らかにすることを目的に、65歳以上の高齢者で生活自立している者47名を対象に、口腔内診察と口腔機能の計測を行い、天然歯列者と義歯使用者の2群における計測値を比較・検討した。口腔内診察では開口障害の有無、咬合状態、舌、および口腔粘膜の異常、残存歯の状況と咬合支持域、義歯の使用や種類について診査した。口腔機能の測定項目は口腔機能低下症の指標である最大舌圧、オーラルディアドコキネシス（ODK)、反復唾液嚥下テスト（RSST)と、口唇閉鎖力、最大開口量の5項目である。その結果、天然歯列群と義歯使用群の間で、最大舌圧、ODK、RSST、口唇閉鎖力および最大開口量の口腔機能について有意差はなかった。これらより、義歯の使用により口腔機能が補完、維持されている可能性が示唆された。 |

Organic changes in the oral cavity due to aging, including tooth loss, are known to affect oral functions such as swallowing and articulation expectancy. On the other hand, It is also becoming clear that even if people lose their some of their own teeth, they can preserve good oral functions and get a sense of satisfaction, or a sense of improvement of their quality of life if they can keep a good bite by wearing appropriate dentures, and that wearing dentures will have a beneficial effect on the whole body. Regarding the effect of denture use on oral function, it has been reported that oral function recovered after improving the fit of dentures in patients with oral hypofunction and implementing health management at the same time. The purpose of this study is to clarify whether the use of dentures has a beneficial effect on the maintenance or improvement of oral functions. The subjects are 47 self-supporting people 65 or older. They were divided into two groups; one is composed of 29 people who are dented, the other 18 people who wear some dentures. Both groups were compared from these two points of view; one is oral examination, which includes trismus, occlusal condition, tongue, oral mucosa, condition of residual teeth, occlusal support area, and the use and type of dentures, the other is oral function, which includes maximal tongue pressure, oral diadochokinesis （ODK), repetitive saliva swallowing test （RSST)（these three items can be indexes of oral hypofunction), oral lip closure, and maximum opening distance （these two items are important features in assessment of oral function). The result shows no significant difference in maximal tongue pressure, ODK, RSST, oral lip closure and maximum opening distance. There were no significant differences in other measurement items This indicates that the use of dentures has a beneficial effect on the maintenance and improvement of oral functions. |
