高齢化に伴う医療ニーズの増大に対応し、医師や歯科医師の判断を待たずに、一定の診療補助業務(特定行為)が実施できる看護師を育成するために、厚生労働省は2015年に特定行為に係る看護師の研修制度を発足した。その研修を修了した者を(通称)特定看護師という。本研究は、発足から10年経過した時点での特定看護師の活動実態を明らかにすることを目的とした文献研究である。文献データベースとして医学中央雑誌webを用いて「特定看護師」をキーワードとし、論文種類を原著論文に絞り込み、最新10年分(2015年～2025年)で検索を行った。テーマに沿った文献を抽出し、15本を分析対象とした。特定行為活動の内容を【特定行為実施状況】【特定行為の実践による効果】【特定看護師に関連した課題】の3つのカテゴリーに分類し内容を整理した結果、【特定行為実施状況】においては、特定看護師に求められる21区分38行為のうち、医師が不在でもタイムリーにリスクの高い医療技術を提供しており、【特定行為の実践による効果】においては、「患者の症状改善」「エビデンスに基づいたマネジメントの実施」「医師の業務負担と働き方改革への貢献」が抽出され、【特定看護師に関連した課題】においては、「特定看護師の効果的な配置」「組織体制の整備」「特定看護師の認知度」に関する課題が抽出された。今後特定看護師がさらに発展し効果的な役割を果たすためには、特定看護師の必要性についての周知を行い、多方面からの課題の解決が必要であることが示された。 |

In response to the increasing medical needs associated with aging, in 2015, the Ministry of Health, Labor and Welfare launched a training program allowing nurses to perform certain medical assistance tasks (specified medical acts: ‘Tokutei acts’) without prior authorization by a physician or dentist. The nurses who have completed the training are designated as specified medical acts nurses: ‘Certified nurses’. The aim of this literature was to review the actual activities of Certified nurses a decade after the launch of the program. We conducted a literature search using the keyword ‘Certified nurses’ in the Central Journal of Medical Science web database to identify relevant original articles published between 2015 and 2025. Literature relevant to the theme was extracted, and 15 articles were included in the analysis. According to activity contents, ‘Tokutei acts’ were classified into three categories: “implementation status of Tokutei acts,” “effects of Tokutei acts,” and “issues related to Certified nurses.” As a result, among the 38 Tokutei acts of 21 divisions required for Certified nurses, “implementation status of Tokutei acts” showed the timely provision of high-risk medical technologies actions even during the physician’s absence. For “effects of Tokutei acts,” “improvement of patient symptoms,” “implementing evidence-based management,” and “contribution to the physician workload and work-style reform” were extracted. For “issues related to Certified nurses,” issues related to “effective placement of specified medical acts nurses,” “development of organizational structure,” and “recognition of Certified nurses” were extracted. To further strengthen the role of Certified nurses, it is necessary to increase awareness of the need for s Certified nurses and address challenges from multiple perspectives. |
